There, as soon as you enter the dragon cave in Garrotte Gorge, the clock starts to count down towards the next respawn, and if you don't collect the goodies scattered on the floor of the innermost cave, you lose them. If you want to collect Ithilgore early, as I usually do, it's close to impossible to survive a journey to the inner cave before the place respawns.
