Various editors, available here and elsewhere, can be used to restructure these games considerably. However, it takes some tinkering to get everything right.The degree of modification varies from mod to mod. Some merely make the game more difficult by increasing monster HP, whereas other MODs seek to reshape the questline and relocate dungeons. Success levels vary, respectively.
