In your first post you said.."The NPC disapeared" and reading on I find that it is the Old Hag who vanished...she has to give you the key to get off the island...Make sure that after you have the guy as an NPC you ask him how to get off the island (press key 5 to enter dialogue) then you go back to her and ask..how to get off the island.
