You go round the back of the castle, kill the basilisk outside and, provided that you have a character with some perception, you will see a red-tinged gap towards the left-hand end of the wall. Hit it a few times to break away all breakable bits of stone, and then Crouch (C on the keyboard) to get through the gap. You then need to find the back door to the Keep itself.
