Did you try going back to the cavern after getting the quest? If that doesn't work google "MM7che" this editor will allow you to edit the map into your inventory.

Are you by any chance using Windows Vista or Windows 7? If so you need to right-click on the editor and choose "Run as administrator" to use it, otherwise the changes you make will show up in the editor but not in the actual game.
... I no longer have MM7 installed so I can't test this out, but if you're certain that you've 'browsed' to the right folder with MM7CHE's file selection window then you might try opening up a Windows Explorer window, navigating to the file, and dragging it into the MM7CHE file selection window's file name field.
The reef chart has always shown up before I get the quest. Did somebody fill up the chest with useless junk? The only other problem I've heard about is that it is possible to miss the right chest.
