Enroth, Erathia, and Jadame was destroyed in the cataclsym caused by the meeting of the Sword of Frost and Armageddon's Blade. HOMM4 picls up (and, I think MM9) on other worlds that the survivors fled to thru portals that suddenly appeared.

This is the story as I understand it and I mak be a little off here and there.
