Anybody with GM alchemy can mix that potion herself/himself  . Otherwise the alchemy shops in Celeste and the Pit often have rejuvenation potions for sale (save before entering in case they don't have what you want). Those are the best places of finding it, otherwise some high level monsters sometimes gives black potions, you can find it in some random chests, in some general stores (cannot recall which ones at the moment) or sometimes in Clankers laboratory.
