When I walk up to those grates, I always just save the game before I try to get the key and then reload and retry until it appears. Typically takes 5 or 6 times but I've never failed to get it eventually. I can't remember the last time I got it on the first try. With the Prince of Thieves I always plan my game so that I can just go directly from the sewers to Castle Stone. Seems odd that the 5 days it takes you to cross the border doesn't seem to matter but any other resting does.
